Child Consent Form

This consent form is to allow the counsellors at Online Counsellor to provide appropriate counselling to a child who is under the age of 18 years. If we don't have approval from a parent or guardian, we will not be able to conduct a session with a child.

You as the parent hereby grant permission for Online Counsellor, a licensed Online Counselling Company, to provide counselling services to my child, (Detailed below), who is under 18 years of age.

I understand and acknowledge the following:

1. I am the legal parent/guardian of the aforementioned child and have the authority to provide consent for their participation in counselling.

2. I am aware that counselling sessions may involve discussions about sensitive topics and emotions.

3. I understand that the information shared during counselling sessions will be kept confidential between the counsellor and my child, except in cases where there are legal obligations to report concerns about the safety and well-being of my child or others.

4. I acknowledge that I may have access to general information about the progress of the counselling sessions, but the specific details of the sessions will remain confidential unless my child decides to share them with me.

5. I agree to provide necessary information to the counsellor to ensure the safety and well-being of my child.

6. I understand that my child has the right to discontinue counselling at any time, and that the counsellor will discuss this decision with them to ensure it is in their best interest.
